Air quality is a care issue, not a news story

From late October into January, Delhi’s air reaches levels that are genuinely dangerous for an older adult with COPD, asthma, heart failure or any chronic respiratory condition. Hospital admissions rise every year in exactly that window, and they are substantially preventable.

What actually helps, in order:

  • Move the daily walk indoors during the worst weeks. A corridor or a room is not as good as a park, and it is far better than stopping moving altogether.
  • Keep one room sealed and filtered — the bedroom, usually — rather than trying to treat the whole house.
  • Watch for the early signs rather than waiting for a crisis: more breathless than usual on the same stairs, a cough that has changed, waking at night short of breath.
  • Do not stop inhalers because the person feels well. This is the season they are for.
  • Have the rescue plan written down and in the house, including which hospital and which medicines to take along.

Winter, which is underestimated every year

Delhi winters are cold enough to cause real harm to a frail older adult, and homes here are built for the summer. Cold rooms stiffen arthritic joints, raise blood pressure and make an early-morning bath a genuine risk for anyone with heart disease.

Practical adjustments: shift bathing to the warmest part of the afternoon, warm the bathroom before use, keep the bedroom heated overnight rather than only at bedtime, and watch for reduced fluid intake — older adults drink far less when it is cold and dehydration in winter is common and easily missed.

NCR is three states and it affects the arrangement

A family in Gurugram, a hospital in South Delhi and a caregiver living in Ghaziabad is an ordinary NCR situation and a difficult daily commute. As in Mumbai, matching is done on real travel rather than distance on a map, and partners are matched within their own part of NCR.

Two other things worth settling early: gated-community and sector entry procedures in Noida and Gurugram, which vary considerably and can delay a caregiver every single morning until somebody registers them properly; and lift access in older South and West Delhi houses, many of which are three or four floors with no lift at all — which decides whether a wheelchair or hospital bed is usable upstairs.

Delhi has among the highest concentrations of tertiary hospitals in the country — AIIMS, Safdarjung, Apollo, Max, Fortis, Medanta in Gurugram — and a good share of our NCR bookings follow a discharge from one of them.

The pattern is familiar: a rushed discharge, eight new medicines, and a family that needs a nurse in the first week to reconcile the list and manage a wound. Alongside that, a caregiver for the daily work of bathing, meals and mobility while strength is rebuilt.
